A minority of patients with Crohn’s disease achieve sustained remission on drug therapy. They are concerned about drug safety and want to know how to optimize their diet. The luminal content drives inflammation and is influenced by diet. These factors, plus evidence for therapeutic benefit with dietary therapy, should stimulate inclusion of diet as part of the therapeutic strategy. A novel and well accepted diet has been developed. This diet leads to equivalent remission rates, an identical decrease in inflammation and similar changes to the microbiome compared to exclusive enteral nutrition.
